Sub: UAN based Claim Form No. 31 (UAN) – Instruction on submission of Declaration Form

(2) Head Office circular of even number dated 04.12.2015 (at Sl. No. 532 of ‘Office orders & circulars’ for 2015-16)

Sir/Madam,

Please refer to the above said subject.

2. It is clarified that Declaration Form can be accepted in case of UAN based claim form without attestation of the employer. Henceforth, the attestation of employers on the Declaration Form would not be required in case of UAN based Claim Form No. 31(UAN).

3. This has approval of the Central P.F. Commissioner.
