As per Income Declaration Scheme, 2016, the declaration of undisclosed income is to be filled in Form 1.

Form 2 is an acknowledgement issued to the declarant by the jurisdictional Principal Commissioner or Commissioner.
Form 3 shall be furnished by the declarant to the jurisdictional Principal Commissioner or Commissioner for the proof of payment of tax, surcharge and penalty made after receiving Form-2.
Form 4 is a certificate granted by the jurisdictional Principal Commissioner or Commissioner after submission of Form 3. For details [Refer Notification 33/2016]
Instructions:
- Attachments cannot exceed 50MB.
- Attachments must be in pdf or zip format.
- Attachments should be scanned with minimum 300dpi.
- Wherever there is a requirement in the Form to submit a signed copy of documents by an Assesse/CA as an attachment, upload the scanned copy of the same documents.

- Appeal should not be dismissed for limitation as delay can be compensated in terms of money
