As per Income Declaration Scheme, 2016, the declaration of undisclosed income is to be filled in Form 1.

Form 2Â is an acknowledgement issued to the declarant by the jurisdictional Principal Commissioner or Commissioner.
Form 3Â shall be furnished by the declarant to the jurisdictional Principal Commissioner or Commissioner for the proof of payment of tax, surcharge and penalty made after receiving Form-2.Â
Form 4 is a certificate granted by the jurisdictional Principal Commissioner or Commissioner after submission of Form 3. For details [Refer Notification 33/2016]
Instructions:
- Attachments cannot exceed 50MB.
- Attachments must be in pdf or zip format.
- Attachments should be scanned with minimum 300dpi.
- Wherever there is a requirement in the Form to submit a signed copy of documents by an Assesse/CA as an attachment, upload the scanned copy of the same documents.
